[Ofsrdc] Membership Information Form

I have made a number of changes to the online membership information
form that many of you used this year.  The form will now remember what
you type, so that you don't have to re-enter it all from scratch year
after year.

One thing that several people have requested is that the online form
indicate the dues and where/when dollar amounts.  To do that, I need to
know what each of the councils charges its clubs for dues.

I would appreciate it if each delegate could send me a short email with
your council's annual dues amount.  I know the TVC number already, so
Paul doesn't have to worry about sending it to me...
